What kind of adult support do you need?

• Transition from children’s services — careful, joined-up planning when a young person is leaving school, changing services or moving into adult social care.
• Residential care — a stable home with specialist, 24-hour support for adults who need a team around them.
• Supported living — your own home and tenancy, with personalised support to build independence and stay in control.
• Community support — flexible help for a few hours a week or towards a specific goal, such as travel, appointments, activities or confidence.

What can adult services help with?

Support is shaped around the person’s strengths, communication, routines, interests and goals. It can include personal care, medication, shopping and budgeting, meal preparation, appointments, education or work, relationships, travel skills, community participation and wellbeing.

How do I choose between supported living and residential care?

Supported living is for people who have their own tenancy or home and want the right level of support to live independently. Residential care provides a staffed home with support available day and night. We listen to what matters and help explore the fit; there is no pressure to decide straight away.

How does an adult services referral start?

We begin with a conversation and a person-centred assessment. We work with the person, family, Local Authority, health professionals and commissioners to understand needs, availability and the right next step.

How are adult services funded?

Funding can come from the person, a Local Authority, the NHS or direct payments. We will explain the options that may apply before anything starts.

How do you keep people safe and in control?

Good care protects rights, dignity, choice and safety. Our support uses person-centred planning, Positive Behaviour Support and regular review to help people build confidence and a life with purpose.
