The optional free-text field is intended for a short summary only. Please do not include detailed medical histories, medication lists, NHS numbers, financial information or other sensitive details in the online form. You can discuss information about health or care needs directly with our team after we have responded.
How we use your information
We use enquiry information to contact you, understand which service may be suitable, arrange or prepare for a care assessment, and keep an appropriate record of the enquiry. We may also use it to respond to a safeguarding concern or comply with a legal obligation where that is necessary.
We do not sell enquiry information or use it to send marketing messages just because you submitted this form. We will only send marketing where we have a lawful basis and any consent required.
